Functional Independent Training (FIT)
The F.I.T. Program is designed to allow the Prostaff Physical Therapy (P.S.P.T.) patient to continue exercising on an independent basis in the P.S.P.T gym.
This is a medically oriented gym atmosphere. An evaluation with a therapist is necessary before beginning the F.I.T. Program, for a one-time charge of $75.00, which may be billed to your insurance. The purpose of the evaluation is to:
- Review past medical history with the client.
- Review the clients existing condition including establishing whether deficits exist that even the client is not aware of.
- Create an initial workout program.
You are advised to consult with your physician when beginning or changing your exercise regime.
The cost for this service is $25.00 for a one month membership or $60.00 for a three month membership. This is NOT billable to your insurance and must be paid in full in advance.
An optional monthly therapist re-assessment of your exercise needs is available for an additional fee of $40.00, NOT billable to your insurance. These re-assessment appointments must be scheduled in advance to assure the therapist's availability.
You may utilize the gym Monday through Friday 7:30a.m. - 5:00p.m., and in Yale Saturday 8:00a.m. - noon, with the supervision of P.S.P.T staff.
Occasionally, P.S.P.T. may be closed for holidays or staff training. The gym will be closed at these times. Watch for posted notification of closing dates and times.
*Note: Your membership with the F.I.T. Program may be renewed as often as you like, unless there is a lapse of activity longer than 3 weeks. If such a lapse occurs, you will be discharged from the F.I.T. Program and will need a new evaluation with a therapist in order to resume your membership. If you do not utilize your membership, your fees paid are NOT refundable.
